What should I enter for the Plan Name?

The Plan Name is ultimately in the employer's discretion, so long as the name does not mislead plan participants and such participants receive all benefit plan component documents that are necessary to satisfy the required elements of an SPD. Many employers simply choose an obvious name such as "The XYZ Company Welfare Benefit Plan."
